    Q: Why are you issuing a voluntary recall for the Firex® model COE 10000 and FADCM 12000 series alarms?

    A: Maple Chase Company, manufacturer of Firex®branded safety products, has announced a voluntary recall in conjunction with the U.S. Consumer Product Safety Commission (CPSC) for the double-chirp issue previously identified in February 2008. This action is to ensure the safety of the occupants of residential properties, including homes, condos, apartments and other facilities in which the affected product may be installed.

    Q: What models are affected?

    A: Maple Chases Firex® model COE 10000 series and model FADCM 12000 series that were manufactured between June 1, 2007 and February 1, 2008. This includes hard-wired with battery back-up, hard-wired, and battery only 10000 and 12000 series models. The majority of units sold were hard-wired with battery back-up, installed by electrical contractors in new or remodeled properties.

    Q: What item numbers are included in the model COE 10000 series and model FADCM 12000 series?

    A: The model COE 10000 series carbon monoxide alarms include item numbers 10000. The model FADCM 12000 series combination smoke/CO alarms include item numbers � 12000, 12000C, 12000-6, 12200, 12220, 12400 and 12400C.

    Q: How can I find out the dates on the units I have?

    A: Affected units include those manufactured between June 1, 2007 and February 1, 2008. Product within this timeframe can be identified by its date code, which appears on the back of individual units. The format of this date code is as follows, for example, product built on June 1, 2007 has a date code reading 2007 JUN01.

    The date code also appears on master cartons in a different format. The affected date codes on master cartons can be identified as 2007152 through and including 2008032. All individual units in a master carton have the same date code.

    Q: How many of the Firex® model COE 10000 and model FADCM 12000 series affected units are out in the market?

    A: Approximately 250,000

    Q: Is the double-chirp issue associated with the Firex® model COE 10000 and model FADCM 12000 series now a safety concern?

    A: Safety is a primary concern of Maple Chase (Firex®). As a result, we have voluntarily issued a recall in conjunction with the U.S. Consumer Product Safety Commission (CPSC) to reduce any safety risks to the end user.

    Q: Has the current decision to issue a voluntary recall of the Firex® model COE 10000 and model FADCM 12000 series been related to any reported personal injuries or deaths?

    A: To date, there have been no reported injuries or deaths related to the double-chirp issue associated with the Firex® model COE 10000 and model FADCM 12000 series products.

    Q: Can I return the recalled product that I have in inventory in exchange for comparable, currently available product?

    A: Yes. The chart below details the comparable Kidde® replacement product available:

    Q: Can I return recalled inventory for immediate credit?

    A:
    Yes. You can return recalled inventory of the Firex® model COE 10000 and model FADCM 12000 series and receive a credit for the purchase price. Please follow your existing Return Material Authorization (RMA) procedure.

    Q: What is Maple Chase communicating to my customers on this issue?

    A: Maple Chase (Firex®) has developed a comprehensive communications plan in conjunction with the CPSC and has issued a formal voluntary recall for the affected products. All affected units will be replaced with comparable Kidde® brand product as soon as possible.

    Q: What should I do if my customers call me to report they have Firex® model COE 10000 and model FADCM 12000 series units installed in a dwelling?

    A: Ask the customer whether the product is installed in an occupied or unoccupied building?

    Occupied: A contractor or builder should call the Recall Center at 888-879-3906 to request the number and types of units needed for occupied dwellings. The replacement product will be shipped directly to the contractor or builder for installation in the occupied dwellings. Contractors and builders should inform homeowners that the replacement units have already been ordered, so the homeowner does not need to call to request product as well.

    Unoccupied:A contractor or builder should call the Recall Center at 888-879-3906 to request the number and types of units needed for occupied dwellings.

    Q: Since I can't use Firex® COs, can I return my Firex® smoke alarms too, so that locations are equipped with the same brand of COs and Smokes, and I'm only carrying one brand?

    A: If you have affected product, you can also request to return smokes on a 2- for-1 ratio and avoid a restocking charge. That is two smoke alarms may be returned for each CO or Smoke/CO combination unit.

    Q: Can I return previously installed units?

    A: Distributor: Please return these to us following our standard warranty procedures.

    Contractors and Builders: Please return these to the distributor you purchased them from for credit, and request they send them to us under warranty.

    Q: Can I receive replacements rather than credit for my return?

    A: We cannot supply direct replacement of these Firex® models. You should return the affected units for credit and then order the substitute Kidde units.

    Q: Will Kidde product interconnect with existing Firex® smokes? Is the adapter required for the interconnect to work?

    A: We have UL approval for interconnection of the most common smoke alarm models. Please note that the adapter is not required for interconnection, but using it avoids having to replace the wiring harness. In the US, the Kidde replacement hardwired CO or CO/smoke combination alarms can be interconnected with the Firex® smoke alarms of models:

    • AD (1018 series)
    • ADC (4518 series) and
    • FADC (4618 and 5000 series),
    In Canada, the Kidde replacement hardwired CO or CO/smoke combination alarms can be interconnected with Firex® smoke alarms of models:
    • G-6 (406C)
    • G-18 (418C)
    • PG (484C)
    • ADC (4518C)

    Q: What can be used to replace an installed Firex® 12000 combination CO/smoke alarm that is interconnected with Firex® smokes?

    A: The Kidde® smoke/CO combo alarm (part 900-0114) is listed for cross-branded interconnection and can be used as a direct replacement for the Firex® 12000.

    Q: What product should be used to replace 6000 series COs When will these products be available again?

    A: This series of product has been discontinued. Please see the table below for suitable Kidde® replacement product:

    Q: How do I handle labor claims/requests from contractors?
    A: Firex® will accept claims from contractors for labor associated with replacing affected products. For timely consideration, these claims should include the following information. Most of this information would typically be on a contractors invoice or service call record:

    • Name of service company
    • Address for mailing payment.
    • Phone number / email for additional information if required.
    • Name of service person(s)
    • Service date(s)
    • Service description single or multiple visits, occupied or unoccupied residences, scheduled or on demand.
    • Number of alarms replaced including model number and date code
    • Service billing man-hours or flat rate
    • Total Charge for service
    • Return of Alarms - preferably, submission of labor claim along with prepaid product return directly to Firex. Alternatively documentation of return to Distributor [name, branch, date, number of units]

    Q: If my Kidde® replacement order doesn't meet my standard minimum order and/or prepaid freight requirements, do I have to pay the freight or minimum order handling fee?

    A: No. As long as your replacement order is offset by an RMA, or a Firex® product order is changed, you will not be charged freight or a minimum order handling fee.
